Livtopia.NET Customer Referral Documentation

 Pre-Register  - FAQ - Terms & Conditions - Code of Conduct  -  Discovery Weekends  -  Documentation

 

The following documentation must be provided prior to your participation or your customers' participation in a referral or in a Discovery Weekend.

For your customer:  

  1. Customer Buyers Agreement.(PDF) must be filled out and signed by the Referred Customer.
  2. The online questionnaire and credit check permission form is here.
For the Referring Consultant:
  1. ICREA/TRC Referral Form (PDF) filled in by the Independent Consultant (USA/Canada).
  2. If you pre-registered prior to June 13th, 2008, you must fax the signed terms and conditions agreement (PDF). Pre-registrants and Registrants after June 13 may agree to the Terms and Conditions electronically.
